A calamitous error by Sheffield United goalkeeper Ian Bennett handed Port Vale a shock 2-1 Carling Cup first-round win at Bramall Lane.
When dealing with an innocuous free-kick from Marc Richards, the United stopper inexplicably let the ball through his hands and could only watch as it trickled over the line to give Richards his second and Vale a famous victory.
Billy Sharp's neat finish had earlier cancelled out Richards' 20th-minute opener and despite dominating large swathes of the match, Sharp's effort was the only time the hosts could beat an inspired Chris Martin in the Vale goal.
Kevin Ellison scored his first goal for Rotherham as they dumped last season's semi-finalists Derby out of the competition with a 2-1 win at Don Valley Stadium.
After some early pressure from the home side which saw Paul Warne shoot just wide, the Championship side went ahead after 11 minutes from their first attack.
Paul Connolly's pass set Lee Croft free on the right and with everyone expecting a shot he pulled the ball back across the edge of the area for Gary Teale to drill home.
Jamie Green's free kick into the penalty area was headed down rather than cleared and Paul Warne pounced for his second goal of the season from 10 yards out a minute before the break.
Nicky Law almost set up Warne for number two just after the break, but with 65 minutes gone the League Two side went in front. Law crossed from the left and Warne laid the ball off for Ellison to fire home.
Gillingham striker Simeon Jackson picked up where he left off at the weekend as he scored the opener in a 2-1 win to help dump Championship side Plymouth out.
Fresh from scoring a hat-trick against Swindon, Jackson netted on 43 minutes before Andy Barcham grabbed a second just two minutes later. Moments after the break Luke Summerfield fired in a freekick for the visitors, but the League One side managed to hold on.
