The Italian striker only left the San Siro this time last year after The Reds agreed to pay £16million to sign him from AC, but one year later, he has failed to impress back in the Premier League and he's already set for a swift return to his homeland.
Struggles in Liverpool
Balotelli only netted twice during his time at Anfield and he was told by Brendan Rodgers before the start of the season that he was surplus to requirements at the club, which led to him training away from the first-team with the rest of the transfer listed players.

However, the striker is now set for a move away with Milan his destination, and speaking about the move, he has confirmed that he was also expecting to return to the Italian club because they have a special place in his heart.
Balotelli was snapped this morning heading to a local hospital in Italy in order to complete his medical, and it's believed a formal deal will be announced within the next few days.
The 25-year-old has already spoken about the move and he has confirmed that he is fit and ready to play for the special club.
“I'm happy to be back, now I’m only waiting to start training,” he said.
“Physically I feel pretty good, I just have to train with the team. I have so much motivation.
“Did I imagine returning to Milan? Yes, I always had Milan in my heart and I always hoped to come back.
“Will I finish my career at Milan? I have to work hard and do well this year, then I'll think about that."
Deal To Be Finalised
If the loan deal can be signed off and agreed to in time, Balotelli could play for Milan this weekend as they take on Empoli, and with agreements met, there should be no problems finalising the deal.
The two parties have agreed on paying half of the striker's wages each, meaning both clubs will be paying £40k-a-week throughout the season.
